Request for Student Make-up Test

Instructors:

  • Please complete one form for each student and exam.
  • Complete this form and submit it before bringing over the exam.
  • Please submit exam and completed form to Kathy Schwartzer, Testing Specialist, Maclay 100A, or in the locked exam box in the mailroom.
  • To assure adequate space in the testing lab, please submit at least 2 days in advance of testing date.
